CEU Information
The University of Texas at Arlington and Loose in the Lab are pleased to be able to offer 5.5 clock hours, or, the equivalent of 0.55 Continuing Education Credits, for teachers who participate in a Loose in the Lab workshop.
A. If you are interested, we need you to do three things before you leave the workshop:
- Print all of the requested information and sign your CEU Request Form. This is included in the materials that you are handed in the morning as you sign in for the workshop.
- The fee for this credit is $ 20. You can pay with cash, we can also process both Visa and MasterCard (no Discover or AMEX please), or you can make out a check to Loose in the Lab.
- Return a completed evaluation form, this was also included in the materials that you received this morning, it is the pale yellow sheet, to the speaker. We will begin accepting CEU applications after lunch.
B. Loose in the Lab will submit a request to UTA for CEU’s by sending in one, large, envelope that contains a typed list of names, your evaluation forms, and a single check for all of the teachers requesting a credit for this workshop. The University only processes the names that are included in our
packet, please do not send anything in on your own. It will get stink eye and be sent back to you. UTA typically mails certificates to the eligible participants in 2-4 weeks. If you have any further questions your speaker would be happy to answer them on one of the breaks during the day. Please note that CEUs will be offered on the day of the workshop ONLY. You will not be able to write or call us to do the paperwork after the fact. Thank You.
